¹ Visual aids are subject to a sub-limit of €260 at the highest level and €180 at the lowest level.
² Preventive dental care and bleaching are subject to a combined sub-limit of €160 at the highest level and €80 at the lowest level.

⁴ Visual aids and refractive surgery are subject to a combined sub-limit of €400 at the highest level and €200 at the lowest level.
